Scope and Contents
Cards contain student’s immunization record with dates when shots occurred. Cards also include parents’ names, address, DOB, home telephone number, emergency phone numbers, miscellaneous notes and name of family physician. For graduates in 1993 and later, see Permanent Record Cards / Cumulative Files
