Student medical records, which include student's name, birth date, medical history, height, weight, test results and immunization information. These records are for students for which the archives do not have a grad or non-grad cum file. For grad and non-grad please check the appropriate series.
NOTE: ACCESS TO ANYTHING OTHER THAN THE OFFICIAL IMMUNIZATION RECORD IS RESTRICTED TO SCHOOL OR STUDENT OF RECORD.
