Scope and Contents

  1. Folders 1 and 2 (1950 – 1969): Lists are arranged in chronological order. Each list is then arranged alphabetically by county, and then alphabetically by individual. Information includes individual’s address and city of residence. Asterisks identify County Health Officers.
  2. Folder 3 (1899 – 1964): Lists are arranged alphabetically, and include the licensee name, year licensed, practice location, and license number.
